Every day, our teams support people to live safely, stay independent and make their own choices about how they live. Our services offer a wide range of roles - including in community, hospital and specialist teams - all working together to support our core aim of supporting people to live their best lives, with no one left behind. Drawing on my own background and experience as a social worker, I’ve seen the positive difference that strengths-based practice and least-restrictive principles can make in people’s lives. That ethos is central to how we work and guides our approach to support and decision-making across adult social care.

Our Vision

Our new operating model is designed to ensure every resident receives timely, consistent, and strengths-based support—so that no one is left behind. By streamlining access, embedding best practice, and connecting people to their communities, we aim to deliver equitable, high-quality outcomes for all.
This transformation is about putting the right people, with the right skills and values, in the right place to make a real difference. The model brings together Connect to Community (C2C) and Area Teams, creating a seamless journey for residents:

  • Connect to Community (C2C) Teams act as the first point of response, offering urgent assessments, safeguarding, and support planning. With a multidisciplinary approach, daily case reviews, and direct links to services like technology enabled care (TEC), reablement, and housing advice, C2C ensures residents get the right help quickly and efficiently.
  • Area Teams provide continuity for those with longer-term or complex needs, building on C2C’s work to deliver person-centred care, Care Act assessments, reviews, and safeguarding. By reducing handoffs and focusing on proactive case management, Area Teams help residents live fulfilling, independent lives.

Together, these teams create a joined-up, resident-focused service that maximises independence, reduces risk, and promotes wellbeing across our communities.
