About our Connect to Community Teams
Connect to Community (C2C) Teams is an innovative, resident-focused team that acts as the first point of response for people needing support. Working in C2C means being part of a dynamic, multidisciplinary team that collaborates daily to provide timely, joined-up care.
You’ll handle a varied and rewarding cases—from giving advice and signposting to arranging urgent support and safeguarding. With access to services like technology enabled care (TEC), reablement, and housing advice, you’ll help residents stay independent and connected to their communities.
What C2C aims to solve:
- Multiple “front doors” that previously caused confusion and delays.
- Long waits for assessments—especially for carers—when early, preventative support could help sooner.
- Inconsistent responses and missed opportunities to connect people with community resources.
- Locality duty teams struggling to prioritise strengths-based, preventative approaches.
By introducing a single, streamlined point of access and a multidisciplinary approach, C2C ensures residents get timely, consistent, and person-centred support from the very first contact.
Our C2C Teams are:
- West 1: Based at Millmead, Guildford
- West 2: Based at Victoria Gate, Woking or The Ashford Centre, Ashford
- East 1: Based at Woodhatch Place, Reigate
- East 2: Based at Dakota in Weybridge or Pipbrook in Dorking

Specialist services within our Connect2Community Teams
Mental Health Services
Our Mental Health Service offers a single point of access for ßÙßÇÂþ» residents with mental health needs and/or their carers to connect with Adult Social Care. We provide initial assessments using a strengths-based approach to understand individual needs and identify the right support. Where ongoing social work intervention is required, referrals are directed to the appropriate teams, and where universal services can meet assessed needs, individuals are signposted accordingly. Our aim is to ensure timely, person-centred support that promotes independence and wellbeing.
Learning Disability, Autism and Transition Services
Our service provides a single point of access for individuals with learning disabilities, autistic people, and those preparing for transition from childhood to adulthood. We carry out initial assessments using a person-centred, strengths-based approach to understand needs and identify appropriate support. Where ongoing social care involvement is required, referrals are directed to the relevant teams; otherwise, individuals are signposted to universal services. Our aim is to ensure timely, inclusive support that promotes independence, choice, and wellbeing.
