What is a Shared Care Record?

The Health and Social Care Act 2012 places a legal requirement on health and social care organisations to share data with other health and social care organisations involved in your care or likely to be involved in your care. A Shared Care Record is a way that health and social care organisations can do this in a secure way by enabling appropriate professionals to access information from different organisations in one place. Using a shared care record helps improve the efficiency of the services and reduces the need for you to repeat your story in each health and social care setting.

Previously the Council was part of the ßÙßÇÂþ»­ Care Record however this has been replaced by the Thames Valley and ßÙßÇÂþ»­ (TVS) Local Health and Care Record (LHCR). TVS use the same data for the same purposes, allowing health and care information to be shared across ßÙßÇÂþ»­, Berkshire, Buckinghamshire, Milton Keynes and Oxfordshire.

Role based access controls are implemented within the shared care record to make certain that only those staff that have a legitimate reason to access your data can do so. In addition to the technical access controls, regular audits are carried out and all partner organisations are bound by an information sharing agreement.

How is the information used?

Your care record is kept in a highly secure system and is only shared under tight controls with qualified and regulated professionals who are directly involved in your care. By sharing this information safely, quickly and securely using the TVS Care Records, health and care workers can access the most current details about you to make the best choices about your care.

Where your information is used to plan and manage local health and social care services, it is always anonymised first so that you cannot be identified from the data.

How we are allowed to use your personal data

The legal basis for processing data within the TVS Care Records is ‘the provision of health or social care services’ General Data Protection Regulations (GDPR) Article 9(2)(h) and GDPR Article 6(1)(e) for a ‘task carried out in the public interest’.

Who is part of the Thames Valley and ßÙßÇÂþ»­ Care Records Partnership?

The partners are Health and Social Care organisations from:

  • ßÙßÇÂþ»­ Heartlands Integrated Care System (ICS) also known as ßÙßÇÂþ»­ Heartlands Health and Care Partnership. This includes ßÙßÇÂþ»­’s Adult and Children’s Social Care Services
  • Frimley Health and Care ICS
  • Buckinghamshire, Oxfordshire and Berkshire West ICS
  • Milton Keynes University Hospital Trust
