Description
The Transition Team at ßÙßÇÂþ» support young people from the age of 14 to 25 with disabilities and their families on their journey to adulthood. They work with the practitioners involved with young people to understand what their ongoing care and support needs might be at when they turn 18.
Based on the information provided the Transition Team will be able to see what support a young person needs, how much this will cost and if eligible for Adult Social care support how much will be funded. Once the support is in place the Transition team regularly review if the support provided meets the needs or a young person and their family and update it if necessary.
