Funded childcare declaration form

Getting funded childcare? Don’t forget this form!

If your child is getting funded early education and childcare, you need to fill out a declaration form.

Your provider will give this to you - if they don't, just ask.

The form lets your provider claim the funded hours for your child.

Important - before you sign

  • Your provider can help you with the form. But it's important you see the form and the number of hours on it before you sign.
  • If you sign too early or give incorrect details, your provider might not get the funding and you might have to pay.

Filling in your declaration form

The form must be completed by the child’s parent or legal guardian.

You need to complete the form after your child’s first funded session. You can do it earlier if you like.

You’ll need to sign and date the form for each funded period:

  • Autumn (1 September to 31 December) – Sign from 1 July
  • Spring (1 January to 31 March) – Sign from 1 December
  • Summer (1 April to 31 August) – Sign from 1 March

You can use the same form all year - just sign and date each funded period.

  • Use your child’s full legal name (no nicknames). Your provider will match this to your ID documents.
  • Read the terms and conditions before you sign
  • Only claim the hours your child is entitled to (15 or 30 hours)
  • Tell your provider if your child goes to another setting, even if it's in a different area

Your child may qualify for extra help if they:

  • Are adopted or in care
  • Have a disability
  • You receive certain benefits

Ask your provider if you're not sure. They will ask for your consent to check if your child qualifies for extra funding through:

This is funding for your provider. It helps them fund extra support, staff training and learning experiences for your child.

Claiming hours for working families or FEET funding?

On the declaration form give:

  • the eligibility code you got when you applied to HM Customs and Revenue (HMRC)
  • your National Insurance number

Can't find your code?  Log in to your HMRC childcare account.

On the declaration form use the  FEET ID. This is the 10-digit code from your approval email.
(format: XX-XYZS-USYZ)

Can’t find your code?  Ask your provider to email: eycommissioningteam@surreycc.gov.uk

If your child qualifies for both hours for working families and 2 year old funding (FEET):

  • Use FEET funding first (FEET funding lasts until the term after your child turns 3 - even if your work situation changes)
  • Then use the hours for working families for the additional 15 hours (from September 2025)
