The LeDeR Programme reviews the lives and deaths of people with learning disabilities and autistic people across England.
The aims of the programme:
Improve care for people with a learning disability and autistic people.
Reduce health inequalities for people with a learning disability and autistic people.
Health inequalities are unfair differences in people’s health.
Prevent people with a learning disability and autistic people from early deaths.

How to report to LeDeR
Anyone can about someone who has died using an online form.
The form should take around 15 minutes to complete.
